Medically Fragile Children and Adult Program
What is it? A chronic physical condition, which results in a prolonged dependency on medical care for which daily skilled (nursing) intervention is medically necessary. Services below:

Supervision: 24-hour supervision or direct assistance to maintain safety, such as turning or repositioning every 2–4 hours to prevent skin breakdown
Monitoring: 24-hour monitoring of a health care plan by a licensed nurse, or continuous monitoring of vital signs
Treatments: Treatments such as feeding tube in the nose or stomach, intravenous therapy, oxygen support or ventilator, urinary catheterization, and cardiorespiratory monitoring
Medication: Medication administration
Specialized equipment: Specialized medical equipment
Trained caregivers: Trained caregivers, such as skilled nurses
